SUMMARY OF ROLE

To provide support to clients, in order to promote independent living within the relevant parameters, as appropriate; clients can be defined as individuals, families or children that might have a range of diverse needs.

ACCOUNTABILITIES

  • To assist clients, by providing support in a flexible and person centred way, delivering practical tasks, which address their needs and aspirations, through drafting ( not authorising) and assisting in the implementation of support/ treatment plans.
  • To draft (not authorise) individual client needs assessment which generates an Outcome Star which shows the distance travelled and outcomes achieved by the clients.
  • To draft (not authorise) individual client risk assessments through effective communication and rapport building with the client.
  • To contribute to the review of support plans and risk assessments in a timely manner, informing all relevant parties of significant changes and alerts as you become aware of them.
  • To work with the team to identify when clients are ready to exit the services, helping them to prepare for independence.
  • To deliver a high quality standard of support that promotes independent living using positive role modelling behaviours. Such as how to behave in meetings, routine appointments and public spaces.
  • To assist Vision18 and the Independent Living Worker level 3 to ensure the maintenance of high quality accommodation, effective rent collection and efficient void and housing management.
  • To ensure a high standard of service delivery is achieved by following 16pluscare’s values, policies, procedures and standards, including safeguarding clients and other people at risk of abuse, compliance with Health and Safety, Lone Working procedures and risk management.
  • To help clients to build and develop strong support networks
  • To undertake basic client involvement as required.
  • To follow best practice guidelines as required
  • To comply with all internal and external audit requirements, including quality standards and accreditation and other commissioner requirements as directed by your manager.
  • To adhere to internal targets and performance measures.
  • To work cooperatively with identified partner agencies to deliver contractual obligations.

KNOWLEDGE, SKILLS & QUALIFICATIONS

  • Commitment to attain NVQ level 2 or equivalent within 2 years.
  • Relevant Experience
  • A willingness to work with vulnerable clients
  • An understanding of Financial, HR, Health & Safety and Equality & Diversity requirements.
  • Willingness to learn and use ICT as appropriate.
  • Ability to achieve realistic targets and deadlines.
  • Ability to work effectively as part of a team and are flexible in approach
  • Effective interpersonal and communication skills.

Performance Measures and KPIs

These will be set in discussion with the job holder.  KPIs will be set around the following areas:

  • Team performance against operational plans
  • Demonstrating corporate values
  • Achievement of service delivery targets

 

Compliance with all internally and externally determined performance standards and legal requirements.
